Step 1
Referral
Participants may be referred through Medi-Cal Managed Care Plans, healthcare providers, or community-based organizations participating in CalAIM programs.

Eligibility Verification
Our intake team reviews eligibility criteria to ensure participants qualify under program guidelines.
Step 3
Nutrition Screening
Participants may receive a nutrition screening or assessment conducted by a Registered Dietitian or qualified nutrition professional.
